It really does mean that you have to keep a good stand on the international politics even while you have to keep in good with your own people for reelection. Warfare means global upset and various nations speaking out against you and ends somewhere around the point where the global community decides on international intervention marked by American peacekeepers. Trying to take, say, North Korea from a nobody to someone of actual importance can be difficult if not impossible. This makes it incredibly fun to play for anyone who enjoys a good nation simulation. Masters of the World is supposed to be way more realistic than Paradox games as they simulate as much as possible.
